We slept on seven mattresses that claim to help with lower back pain and measured how well each one held the spine in line. Three were worth recommending.

What actually matters here

A mattress cannot cure back pain, and any site telling you otherwise is selling something. What a good mattress can do is stop making it worse: hold the pelvis level with the shoulders, let the lumbar curve stay supported instead of sagging into a hammock, and allow you to turn over without bracing.

In our testing, that consistently meant a medium-firm hybrid with a supportive coil base. Every mattress below scored above 8.5 on spinal alignment for at least two of our three testers.

How to choose, in four decisions

Firmness is relative to your weight

The same mattress measures differently under a 135 lb sleeper and a 240 lb one. A medium-firm 6 will feel like a 7 to a light sleeper and a 5 to a heavy one. When a review gives a single firmness number, ask who was lying on it. Ours are measured on a compression gauge and cross-checked against three testers at 135, 175 and 240 lb.

Support matters more than the comfort layer

The layer you feel is not the layer doing the work. Lower-back pain usually comes from the pelvis dropping below the line of the shoulders, and what stops that is the support core — coils or high-density base foam — not the pillow top. This is why coil-on-coil constructions keep winning this category.

Zoned lumbar support is not always marketing

Plenty of brands print "zoned" on the box and change nothing. A real zoned layer uses a firmer coil gauge or a denser foam under the hips, and you can feel the transition with your hand. The Helix Luxe range does it properly; several cheaper mattresses we tested do not.

Give it thirty nights before you judge

Almost everyone coming off a soft or sagging mattress reports discomfort in week one on a properly supportive bed. Your body has adapted to the old shape. Every mattress on this page has a trial of at least 120 nights, which is enough to be certain.

Frequently asked questions

Is a firm or soft mattress better for back pain?
Medium-firm, in almost every case. A 2015 review in the Journal of Physical Therapy Science and a large 2021 randomised trial both found medium-firm outperformed firm for chronic low back pain. Very firm mattresses leave the lumbar curve unsupported; very soft ones let the pelvis drop. Our three recommendations measure between 6 and 6.5 on a ten-point gauge.
Can a new mattress make back pain worse?
Temporarily, yes. Moving from a sagging mattress to a supportive one changes the position your spine holds for eight hours a night, and the adjustment commonly takes two to four weeks. If pain is worsening after a month, the mattress is wrong for you — use the trial period.
Do mattress toppers help with back pain?
A topper can add cushioning to a mattress that is too firm. It cannot add support to one that is too soft, because the sag is underneath it. If your hips are sinking, a topper will not help and a new support core will.
How often should I replace a mattress if I have back pain?
Watch for sag rather than the calendar. Lay a straight edge across the mattress; if you can see more than about three-quarters of an inch of dip where you sleep, the support core has gone and it is time. On a good hybrid that is usually eight to twelve years.
