What actually matters here

Sleeping on your side puts your entire body weight through two small contact areas: the point of the shoulder and the greater trochanter of the hip. A mattress that is too firm concentrates pressure there, which is why side sleepers wake with a numb arm or an aching hip. A mattress that is too soft solves that and creates a new problem, letting the pelvis drop out of line.

The mattresses that win this category do both things at once, usually with a zoned support layer: softer under the shoulders, firmer under the hips. We measure peak pressure on a sensor mat and spinal alignment from side-on photographs, and we only recommend mattresses that score well on both.

How to choose, in four decisions

Weight decides firmness more than preference does

Our 135 lb tester and our 240 lb tester almost never agree. The same medium mattress gave the lighter tester excellent pressure relief and let the heavier one’s hips drop 2.4 inches. If you weigh under about 150 lb, buy softer than the reviews suggest. Over about 230 lb, buy firmer.

Look for zoning, then verify it

A genuine zoned layer changes coil gauge or foam density under the hips. Press down firmly with a flat palm at shoulder height and again at hip height — you should feel the difference through the cover. If you cannot, the zoning is a print on the box.

Shoulder pressure shows up in week one

Unlike back support, which takes a fortnight to judge, pressure problems announce themselves quickly. If you are waking with a dead arm on night four, the mattress is too firm for your shoulder and thirty more nights will not fix it.

Your pillow is half the problem

Side sleeping needs a taller pillow than back sleeping — enough loft to fill the gap between the mattress and your ear without tilting your head. Getting a mattress right and leaving a flat pillow in place will keep the neck pain and convince you the mattress failed.

What firmness is best for side sleepers?
Medium, around 5 to 6 on a ten-point scale, for most people between 150 and 230 lb. Under 150 lb, a soft 4 will usually relieve more pressure without alignment problems. Over 230 lb, a medium-firm 6.5 is generally the safer choice because a softer surface lets the hips drop.
Is memory foam or hybrid better for side sleepers?
A hybrid with a memory-foam comfort layer, in our testing. Pure memory foam relieves pressure superbly and supports the pelvis poorly; a coil unit underneath fixes the second problem without giving up much of the first. Our top pick in this category is exactly that construction.
Why does my shoulder hurt on a new mattress?
Usually because the comfort layer is too thin or too firm to let the shoulder sink in, so your body weight concentrates on a few square inches. It can also be a pillow that is too flat, which pushes the shoulder forward. If a mattress is genuinely too firm at the shoulder, a two-inch topper will help; if the support core is wrong, it will not.
